Schaumburg Township Passport Book
Applications
(Effective February 1, 2008)
The Township of Schaumburg is a Passport Acceptance Facility. Applications are
processed Monday thru Friday, 8:30 AM to 4:30 PM in the Clerk’s Office. If you
need
additional information, please call us at (847) 884-0030.
If you are traveling out of the country by air, you are required to obtain a
U.S.
Passport Book. Unlike a U.S. Passport Card, the U.S. Passport Book will allow
you to
travel by land, air and sea.
The processing times for the U.S. Passport Book are the same as the U.S.
Passport
card. Please allow yourself enough time to obtain a U.S. Passport Book. The
current
processing time is 4-6 weeks for Routine and 3 weeks for Expedite.
You will need to bring the following to the Township:
- The completed DS-11 application (unsigned). If your application is signed
when you arrive you will be asked to start over.
- A certified copy of your birth certificate issued by the county or state
you
were born in. Naturalized citizens must provide their original certificate of
naturalization.
- If you have an expired passport book/card, more than 15 years old from the
issue date, you will be required to submit it with your application. If your
previous passport is lost or stolen, you will be required to fill out a DS-64
form along with your DS-11 application.
- Proof of identity: Valid drivers license or state ID.
- Passport fees are $75.00 for adults and $60.00 for children 15 years
of age and under and must be paid in the form of check or money
order. An additional $25.00 processing fee will be charged per
application payable by cash or check to the Township of Schaumburg.
- Two passport photos, each 2" x 2".
If you are planning to expedite your application, there will be an additional
charge of
$60.00 (adult and children), plus a charge for overnight mailing (optional).Children fifteen (15) years of age and under must be accompanied by both
parents
or submit an original legal document stating sole legal custody has been
granted to
one parent. If the other parent cannot be present, you must submit a notarized
DS-3053 parental consent form with the child’s application.
Children sixteen (16) years of age and over (until the age of 18) must be
accompanied by one parent.

*Expedite Fees are not included in this amount. Applications for an Expedited
U.S.
Passport Card will not be accepted until further notice from the Department of
